St Louis Community Care Packages support older people to enjoy quality of life while remaining in their own homes.

Who is Eligible?

• Those people assessed by an Aged care Assessment Team (A.C.A.T.) as requiring low level support and Suffering Dementia.
• People with complex care needs (medical, social, physical or emotional).

Fees

There is a contribution fee of up to 17.5% of the Aged Pension (this may be negotiable depending on individual financial situations).

Services Provided

A Community care package will suit the individual needs of each person and may include support with the following:

• Personal care and hygiene
• Home help (cleaning, laundry, gardening etc)
• Transport assistance (appointments, outings, shopping)
• Meal preparation
• Medical support
• Pet care
• Personal and financial affairs
• Respite
• Advocacy
• Social Support

St Louis Community Care Packages is an expansion of the services offered by St Louis Nursing Home ... a private organisation that has been providing aged care services to the community since 1946.

St Louis recognises that elderly people have the right to determine what their care needs are and should be supported and actively involved in all aspects of service provision. Care workers are chosen accordingly.
