Domestic Assistance - Cleaning, vacuuming, mopping, laundry - all to keep you safe
Grocery shopping and errands - Grocery shopping, general errands and putting groceries away.
Meal preparation - Support to prepare and cook meals, assist with diet and health, and maintain hydration.
Transport for older people - Safe assistance for appointments, medical, social, health, community and shopping.
Home maintenance and modifications - Keep you and your home safe - installing handrails and other modifications if required.
Personal care and services - Support you with dressing, grooming, showering, toileting, and continence management.
Home nursing - Our Clinical Care Nurses are trained to support you.
Medication support - Medication reminders and storing medications safely in your home.
Pain management - Support programs to manage pain.
Home Physiotherapy and Allied Health - Physiotherapist and Allied Health team can help you.

Dementia care - Low and high-level support for dementia care to live safely at home.
Hospital-to-home care - Our clinical nurse team provide hospital to home support.
Activities of daily living - Personal support with dignity for all daily activities.
Social support - Social support for your lifestyle, interest and community connections.
Exercise - Allied Health approved activities may include; swimming, walking or hydrotherapy programs.
Nutrition and cooking - Support for your health and well-being through nutrition and cooking.
Respite - Lifestyle, well-being, day therapy, social and community support.
Community Activities - Support for social, lifestyle, well-being and community connection.
Spirituality, religion and culture - Stay connected to your spiritual needs at home or in the community.
A home Care Package is government-funded support allocated for older people to access coordinated care with a support plan. This helps you to meet you needs, goals, and choices to live safely and independently at home.
Government Funded Home Care Package
Funding is accessed through an assessment with My Aged Care. There are four levels of Home Care Package funding:
Level 1 basic care needs
Level 2 low-level care needs
Level 3 intermediate care needs
Level 4 high-level care needs
If your care needs change, you can apply to have your funding support increased.
